Lifeline Senior Advocates provides independent senior monitoring and health advocacy services for families who want consistent oversight and hands on support after placement. We work alongside facilities and healthcare providers to ensure your loved one receives attentive care, clear communication, and proper follow through.

We provide regular in person visits to assisted living, memory care, and skilled nursing facilities to observe care quality, safety, cleanliness, engagement, and staff responsiveness. Concerns are identified early and communicated clearly to families.
We attend medical appointments when needed, assist with transportation when appropriate, help clarify diagnoses and treatment plans, review medications with providers, and ensure follow up recommendations are understood and implemented.
We coordinate directly with facility staff, physicians, specialists, pharmacies, and care teams to ensure care plans are followed, medications are managed correctly, appointments are scheduled, and changes in condition or treatment are communicated clearly.
We provide support with understanding medication routines, changes, and potential concerns by coordinating with healthcare providers and facility staff. We do not administer medications, but we help ensure medications are organized, monitored, and reviewed appropriately.
Families receive clear summaries and updates following visits, appointments, hospitalizations, or changes in condition. This ensures families remain informed and confident in their loved one’s care, especially when they are unable to be present.
If your loved one is hospitalized or sent to the emergency room, we support care transitions by coordinating communication between medical teams and the facility to ensure discharge instructions and follow up care are carried out consistently.
Facilities provide care for many residents at once. As independent advocates, our focus is on your loved one alone. We work for families, not facilities, allowing us to ask questions, follow up persistently, and advocate without conflicts of interest.
Our involvement helps prevent miscommunication, missed follow ups, and gaps in care that can occur when families are stretched thin or managing care from a distance.
Services are typically offered through a monthly advocacy plan or retainer based on the level of support needed.
