• Initial Assessment: A comprehensive review of medical history, current healthcare needs, and care coordination requirements.

• Personalized Healthcare Plan: A tailored plan addressing both short- and long-term healthcare needs.

• Advocacy Services: Up to 10 hours per month of support, including:
  • Attending medical appointments.
  • Communicating with healthcare providers.
  • Coordinating care among specialists.

• Progress Reports: Detailed updates on medical status and recommendations for continued care.

 Additional advocacy hours available at $135/hour. 

Our Nursing Home Monitoring Package ensures your loved one receives high-quality care in a nursing home setting  while offering flexibility to meet your family’s budget and needs. This package includes:

• Initial Assessment: Onsite evaluation to review your loved one’s care plan, health condition, and specific concerns. A personalized monitoring plan will be developed to outline actionable goals.

• Regular Visits: Up to 4 visits per month (weekly or bi-weekly) to monitor care and address concerns. Visits typically last 1 to 1.5 hours.

• Staff Communication: Direct advocacy with nursing home staff and physicians to ensure your loved one’s needs are met.

• Care Reports: Detailed updates on your loved one’s condition and quality of care.

Visit frequency can be adjusted based on your budget and preferences.  Additional visits available at $200/visit. 

Our Insurance Assistance Package simplifies the complexities of insurance claims, Medicare, Medicaid, and coverage issues. This package includes:

• Initial Assessment: A thorough review of insurance policies and coverage needs.

• Personalized Plan: A customized healthcare plan to maximize benefits and address coverage gaps.

• Insurance Support: Up to 10 hours per month of expert assistance, including:
  • Filing claims and resolving disputes.
  • Communicating with insurance companies.
  • Maximizing Medicare and Medicaid benefits.

 Additional assistance hours available at $150/hour.
